NOTE: We can not rush in the care of our patients - and may be running behind. Many of our patients have complex problems and require extra time and care. Bring a book or a laptop - some of our offices offer wireless access. Please prepare for this by bringing: Health Insurance Card / Information A friend or family member to take notes / help with questions A list of all medications you are taking A list of your questions /concerns/ past medical history and surgeries A copy of your XRAYS / CT scans (not just the reports - please get the actual films) Copies of records from doctors related to your sinuses / nose / allergies / lungs.
